A guide to enabling behaviour change for health and wellbeing using person- and community-centred approaches
"This guide is for people who support those living with long-term conditions, their carers, families or communities. It summarises practical ways to support people to self manage effectively using person and community-centred approaches. Many of these activities are useful also for people who work to prevent the development of long-term conditions in the first place."

Transforming our health care system: Ten priorities for commissioners
"We have identified ten priorities for action. A striking feature of all the priorities is the degree to which they call for change within primary care and the way in which primary care relates to the rest of the system. "

The dynamics of quality: a national panel study of evidence-based standards
"People with long-term health conditions do not always receive the health care they need, and so may suffer avoidable poor health. We aimed to find out if the quality of health care received by people in England had improved over 6 years, for four common long-term health conditions: cardiovascular disease (including heart disease and high blood pressure treatment), depression, diabetes and osteoarthritis."

Reducing Care Utilisation through Self-management Interventions
"Very few self-management support interventions achieve reductions in utilisation while compromising patient outcomes. Evidence for significant reductions in utilisation were strongest for respiratory disorders and cardiac disorders."

Delivering High Value Care Pathways
"In this scenario we examine a typical long term condition pathway, comparing a sub-optimal but typical scenario against an ideal pathway. At each stage we have modelled the costs of care, both financial to the commissioner but also the impact and cost on the patient, family and their contribution to society. We show how the Right Care methodology can help clinicians and commissioners to improve the value of the care pathway."

Changes in costs and effects after the implementation of disease management programs
"After one year we have found indications of improvements in level of integrated care for cardiovascular risk patients and lifestyle indicators for all diseases, but in none of the diseases we have found indications of cost savings due to disease management programs. However, it is likely that it takes more time before the improvements in care lead to reductions in complications and hospitalizations."

Rethinking the patient
"Burden of Treatment Theory is oriented to understanding how capacity for action interacts with the work that stems from healthcare. Burden of Treatment Theory is a structural model that focuses on the work that patients and their networks do. It thus helps us understand variations in healthcare utilization and adherence in different healthcare settings and clinical contexts."

Management of long term conditions
"Our analysis concluded that without unprecedented, sustained increases in health service productivity, including more effective management of chronic conditions, funding for the NHS in England will need to increase in real terms between 2015/16 and 2021/22 to avoid cuts to the service or a fall in quality."
