"The report identifies 10 areas where there is particular scope for improvement. These span the full range of health system activities, illustrating that change is needed across the system. Commissioners and providers can use these 10 areas as a guide to identify where some of the most significant opportunities for quality improvement and cost control lie."

Transforming our health care system: Ten priorities for commissioners
"We have identified ten priorities for action. A striking feature of all the priorities is the degree to which they call for change within primary care and the way in which primary care relates to the rest of the system. "

New care models explained: How the NHS can successfully integrate care
"Multispecialty community providers (or MCPs) and the primary and acute care services (or PACS) are new types of delivery models, integrating hitherto relatively poorly coordinated care providers into new and efficient organisations or alliances capable of delivering on the “triple aim” – a better patient experience, better population health and more efficient use of resources."

Population health systems: Going beyond integrated care
"The central purpose of this paper is to challenge those involved in integrated care and public health to ‘join up the dots’. This challenge recognises that population health is affected by a wide range of influences across society and within communities. Improving population health is not just the responsibility of health and social care services, or of public health professionals. Instead, we argue that it requires co-ordinated efforts across population health systems."

Joining up health and social care personal budgets: Key points on implementation
"Joint health and care personal budgets offer a potential model for delivering integrated care from the bottom up, at the level of the individual person. This briefing outlines the key themes from a workshop for senior professionals on joining up personal budgets, held at the King’s Fund. It also sets out a series of practical approaches to implementation that might be considered by local
leaders as they develop their approaches to integrating personal budgets."

Inclusive integration: How whole person care can work for adults with disabilities
"This report provides an empirical evidence base to demonstrate how whole person care should meet the needs of working age disabled adults. It explores how integrated care outcomes for working-age disabled adults might be formulated to take account of wider health inequalities within a social model of disability."

"Capitated payment can offer significant benefits to patients but there are some risks. For the approach to deliver its full potential benefits to patients and to the health system as a whole, commissioners and providers need to agree on mechanisms for mitigating the risks."

Changes in costs and effects after the implementation of disease management programs
"After one year we have found indications of improvements in level of integrated care for cardiovascular risk patients and lifestyle indicators for all diseases, but in none of the diseases we have found indications of cost savings due to disease management programs. However, it is likely that it takes more time before the improvements in care lead to reductions in complications and hospitalizations."

Commissioning and contracting for integrated care
"Many clinical commissioning groups (CCGs) have started to develop novel contracting and commissioning tools to drive more transformational and sustainable service integration. There is a range of contractual models being discussed and implemented across England. In this paper, we describe two broad frameworks that are currently being developed in five areas of the country to stimulate more integrated models of care – a prime contract and an alliance contract."

System leadership: Lessons and learning
"A consensus is emerging in England around the concept of ‘integrated care’ as the best hope for a sustainable NHS. For leaders in the health care system, this represents an immense challenge. Leading across complex interdependent systems of care is a new and different role, undertaken alongside the already difficult task of leading successful institutions."
