Developing a Complex Care Case Management Service within Primary Care

"Analysis of the Slough CCG patient population demonstrated that 5% (7,500) of people consume 43% of healthcare resources.  In an attempt to realise cost savings and improve quality of service, efforts were made to target those at higher risk of an emergency admission or A&E attendance."

A perfect storm: an impossible climate for NHS providers’ finances?

" The failure to spread good practice is not a result of those in the NHS wilfully ignoring the opportunities to improve. Rather it reflects the inherent challenges of transforming a service as complex as health care."

BRAFV600E mutation testing for thyroid cancer

"The BRAF V600E mutation test is more than 99% specific for thyroid cancer, so a positive result means that it is more than 99% likely that the patient has a malignancy and the entire thyroid can be removed in 1 operation rather than 2. This reduces the risks of complications and shortens the total time spent in hospital and recovery."

Cost and risk management in critical care: reducing the use of colloids

"In the past, colloid fluid therapy was used as the primary resuscitation fluid in critically ill people. Increasing evidence ... suggests that crystalloid fluids are equivalent in effectiveness and cost considerably less. This project aimed to reduce colloid use by removing them from the clinical area and making them available by consultant prescription only."

A Medicines Optimisation Service

"The service was developed as a result of a pilot in one practice who used innovation funding to employ a pharmacist to improve their management of medicines. The idea came from the GPs who felt they were always rushed when looking at discharge medicines and this task would be more efficiently and effectively undertaken by a pharmacist. "

Stopping perioperative antibiotics for post-tonsillectomy morbidity

"Considering the implications for development of antibiotic resistance with increased antibiotic use and the lack of clinical benefits demonstrated by this review, routine perioperative antibiotic prescription for tonsillectomy is not recommended."

Mobile clinics for chronic stable glaucoma management

"This initiative aims to improve productivity by reducing demand for appointments from low risk patients in hospital eye clinics, allowing the hospital to see more complex cases. The patient experience should improve because the number of appointments required for testing is reduced and the clinic is brought to locations nearer patients’ homes."

Antigen-specific active immunotherapy for ovarian cancer

"Antigen-specific active immunotherapy for ovarian cancer is not supported by sufficient good quality evidence. Consideration could be given to using it only within the context of a research or audit project.

Reducing or stopping antigen-specific active immunotherapy for ovarian cancer is likely to improve the quality of patient care by reducing exposure to unproven therapies and result in productivity savings."

Bed rest and posture for preventing post-dural puncture headache

"There are no clear benefits associated with either bed rest or fluid supplementation. It is not current practice for patients to be kept in hospital strictly for bed rest and so this review would not result in the freeing up of beds but there may be cash savings from the reduced use of intravenous fluids."

Pressure ulcers: NICE clinical audit tools

NICE have published two clinical audit tools for pressure ulcer prevention, one for paediatric care and one for adults.

"Clinical audit tools are Excel spreadsheets developed to help with clinical audit. They contain clinical audit standards, data collection sheets, a clinical audit report an action plan template and a printable data collection form.  When data is entered into the data collection sheets the audit results are automatically displayed in the audit report."
