Our study identifies sub-optimal discharge communication at the OUH Trust, which is likely to be reflected in other trusts. In particular, the areas of concern include medication reconciliation and follow-up plans."

QIPP (Quality, Innovation, Productivity and Prevention) aims to help NHS organisations deliver higher quality care and operate more efficiently and effectively. The Royal Free London NHS Foundation Trust has set up a programme to implement QIPP across the trust.
