Needs based palliative care commissioning for children, young people and their families

"This document is designed to assist commissioners in identifying the types of
services they should consider when commissioning provision for children and young
people with life-limiting and / or life-threatening conditions. Any commissioning
should be based on an individual’s holistic needs of support and care, and those of
their families. These will include health, social care and general support, recognising
that the emerging model for commissioning is one with a focus on localisation."
